RISK MANAGEMENT DIRECTIVE - PATIENT TRANSPORT
ISSUE: We had a recent case where a VisionQwest Nurse was acting as the charge nurse for the duty shift. In the course of her duty a patient needed medical transport. The patient was a heart care patient and needed immediate transport to another medical facility in which the family had arranged for. The Nurse called a private ambulance service to transport the patient to the medical facility, however the transport service was very late in attending to the patient. The patient's family got very upset and asked why the Nurse had not called 911 since there was heart patient that needed attention. The Nurse thinking that since she had already called for a private ambulance transport she thought her job was completed and 911 was not needed. This outraged the family and a formal complaint was filed with the California Department of Health Services. The CDHS wrote the facility up with a citation for not providing emergency transport to a patient with a heart condition that was scheduled for a higher-level medical facility. This will cost our client facility between $5,000 and $100,000 and will now cost VisionQwest.
DIRECTIVE: If any patient has a medical history of heart conditions or any other tramatic high-risk health condition that would require ambulance transport to another medical facility and the ambulance is slow to respond, 911 is to be called. It has to be a prompt decission by the acting Charge Nurse. If you are the acting Charge Nurse you will make the prompt decision and then notify the Director of Nursing of your decision. Also, you must file a incident report to VisionQwest within 8 hours of the incident. This is standard procedure on all incidents that happen at one of our client facilities. Within 8 hours of any medical or administrative incident, VisionQwest must be notified in writing of the issues of the incident. If we need to be called to the facility to resolve the issue, we must be notified immediately.

In line with this, you may now download a form called the INCIDENT REPORT form to use to notify VisionQwest if such event arises and for other documentation purposes.

Notice - 4: Care Giver Referral Dollars
Effective today all nursing staff will receive a $100 referral fee if they refer their facilities patients to VisionQwest Healthcare Services and they become a patient of the firm. You have patients at your facilities that are released and go home, some of those individuals need caregivers in their homes. Please refer them to our firm, we will assign them to a Case Manager and once they are set up as a patient you will receive a $100 referral fee.

Notice - 1: Transportation Service
The management of VisionQwest Healthcare Services would like to inform you that transportation service is available to our registry nurses effective June 16, 2006. The service is to accommodate our registry nurses to travel from home to work, work to medical appointment (made by the registry nurse or for the patient), emergency situation that needs immediate transport, and from work to home.

Notice - 3: Employment Status & Outside Employment
If you are an international Nurse, LVN or other healthcare worker (one who is here on a special visa and is being petitioned by our client) you are not allowed to take on outside work assignments with other facilities that is not a member of our client overall facilities. By being petitioned for legal work status with our client you are under very specific work guidelines. If you work for another company you are putting our client in legal jeopardy, as well as VisionQwest. This will not be tolerated and you will be asked to leave the company and your petition will be revoked leaving you unable to work legally in this country or in the Nursing or Healthcare industry. If you want added work you must contact us and we will find you another client facility that is covered in your petition to increase your work hours.
